Applying for Residency
If you would like to be part of the BallyCara family we need you to provide us with the following information:
- A completed BallyCara Application for Admissions Form – available below
- A copy of the My Support Plan or referral number provided by the Aged Care Assessment Team (ACAT) or the older 9-page Aged Care Client Record (ACCR)
- A certified copy of the Enduring Power of Attorney Documentation (all pages of booklet)
- A copy of the letter received from the Department of Human Services or Veterans’ Affairs after the “Combined Assets and Income Assessment for Residential Care” has been processed.
On receipt of ALL OF THESE COMPLETED DOCUMENTS, you will be placed on our waiting list.
Frequently Asked Questions
How long is the waiting list for Residential Care? At BallyCara we approach vacancies with a “who is most in need” basis. We ask that you let the BallyCara staff know when you have any major health changes so that we have the most up-to-date information. Likewise, if you find placement elsewhere and wish to be taken off our waiting list, please let us know.
Who is the doctor at BallyCara? When you come into permanent Residential Care, we ask that you talk to your doctor and see if they would like to visit BallyCara on a regular basis. If they cannot visit regularly, then we are happy to recommend one of the doctors that regularly visits BallyCara.
How often can family and friends visit? BallyCara has no set visiting hours for family and friends. We are more than happy for family and friends to visits as often as they wish.
